Over 600 patients from Bellin Health Family Medical Center have been affected by a data breach. The breach was discovered by Green Bay Law Enforcement and was reported to Bellin in mid March 2007. Individuals affected have been notified as of May 3, 2007 via letter. The letter indicates the nature of the breach and offers resources for those involved to request a credit report and place fraud alerts in their accounts. Children involved however should not have credit reports. If a request is made for a credit report for a child and one exists it may indicate fraud has occurred. The guardian should contact The Office of Privacy Protection for assistance. The guardians of the children affected can request a social security statement for the child to determine whether abuse has occurred.
