Approximately 400 patient records (215 being WI residents) Patients gave social security numbers to hospital and a billing service (Hospital Billing and Collection Service “HBCS”) worker gave them to an alleged thief. 133 of the 418 accounts were used by the individual.  HBCS was notified of the breach on May 24, 2006 and notified patients in writing on June 16, 2006.

970,000 individuals (including some employees of the Stoughton School District in Wisconsin) Two laptop computers and a server were stolen from the Midwest regional office of the American International Group and officials say personal information of nearly 1 million people were taken.
