Details

Approximately 650 patients of Bellin Health were affected.

Invoices mailed to the affected individuals contained social security numbers that may have been viewable through the window envelope that contained them. The vendor that processes billing for Bellin notified them of the incident. Letters were mailed to all those affected on December 20, 2007 and offer credit monitoring services through TransUnion.
