Details

More than 28,000 customers mostly in Ohio. The consumers affected are anyone that purchased an individual health plan independently of an employer or employees of Nationwide.

28,279 consumer records were stolen from Concentra Perferred Systems offices. Concentra is a subcontractor that audits hospital stays charges for Nationwide. The company's auto, life and homeowners insurance was not affected.

Details

Potentially millions of consumers who shop at any of the TJX affiliated stores and pay using credit card or debit card. Individuals who have provided personal information for the purpose of merchandise return are also vulnerable.

Hackers broke into a system that stores financial information for consumers in the US, Puerto Rico. TJX Co. is still investigating how widespread the data breach is and who it actually affects.

Details
80,000 MoneyGram customers with 6 Wisconsin residents affected. Wisconsin residents have been notified and offered one year of credit monitoring services free. MoneyGram's server where personal information was stored was accessed by a hacker through the web. All three credit reporting agencies have been notified.
