Details

There was a breach of information affecting approximately 103,000 doctors who are registered with Health Net Federal Services or are in Health Net Federal Services’ network. This does affect some doctors in Wisconsin.

Health Net Federal Services is located in California. Health Net Federal Services had information, which included names and social security numbers of doctors, that was able to be accessed through an unsecured internet site. Once Health Net Federal Service found out about the problem, they took steps to protect the information and sent letters to all doctors affected.

Details

More than 200 faculty and staff members of the UW Dept of IT.

Personal information of those affected was discovered by another employee on a Web-based database. The database was intended to track purchase information for statistical purposes.

Wisconsin Department of Revenue

Who’s affected
Details

Approximately 5,000 residents in northeastern Wisconsin. The mailing was sent to tax payers in the following communities: Freedom, Kaukauna, Keshena, Kimberly, Krakow, Lakewood, Lena, Little Chute, Little Saumico and Marinette.

The taxpayers affected received Department of Revenue 1099-G forms, which report taxpayers’ state tax refund for federal income taxes. The documents were improperly folded by a machine at the Dept. of Administration and as a result showed the social security number through the envelope window.
Letters were sent to those affected on Monday, January 14, 2008.  The federal government requires the social security number be printed on the form. Free credit monitoring for a year will be offered to those affected.

Details

Individuals affected are a segment of the elderly, disabled persons and lower income.

A quarterly mailing was printed with approximately 260,000 individual’s social security numbers visible. The vendor involved noticed the error and did not mail the entire batch of 435,000 mailings. A notification letter will be sent out to the affected individuals later next week and will offer free credit monitoring services from Equifax.
